IPA Lite Onboard

Once IPA has the following information from the client & all info is attached in Hawksoft and labeled, fill out form entirely and click send. This form will be attached in Hawksoft CMS and a checklist will be completed. If agent is also writing additional lines of business such as Commercial, Life, Health or Disability be sure to fill out those seperate onboarding forms.

Items Needed:

* Completed Onboard Form

* All State Licenses OR PDF of License Info

* E&O

* Signed Contract-via docusign

* Signed Contract Addendums-via docusign

* Any additional carrier signed forms ie Kemper, Infinity Etc 

*W9-via docusign

*EFT if filled out

CMIT has created IPA Email address and credentials are attached inside of Hawksoft*
We will be utilizing the agents professional email address (gmail, yahoo and personal emails not accepted)*
Completed opening call with agent *
Call and introduce yourself to the new agent. Call to the producer to let them know that we are beginning their health insurance onboard process. *Collect the DOB/SS as needed *Let agent/agency know we have sent their BGA onboarding form via DocuSign *Request agent to complete and sign in order for us to complete the process.

Check box for forms that are attached and labeled in Hawksoft client file (Capitalize all letters of labelled documents) example: MARYLAND INSURANCE LICENSE; SIGNED EFT FORM*
Check boxes of all info that is added in the clients Hawksoft file (Under personal info)*
All items must be attached in client file
Black Gould (BGA) *
Email Lisa Duran [email protected] a copy of all states licenses, the completed sub-producer form, all the producers contact information (address, phone and email). From there, the BGA team will contact the producer to provide credentials to the system and to contract them with each carrier they want to represent in the Individual Health, Medicare, Small Group and Large Group arenas. BGA will oversee all further correspondence with the producers to ensure they are contracted, quoting and to answer any questions.
